RAPIDLY INDENTIFY CORE MEASURES CASES

With IWT's data mining technology, CORE MEASURES can identify potential cases in real time, while there is still time to provide mandated care. Our software examines clinical data and automatically recognizes situations that fit into specific core measures cases, or staff can manually flag a core measures case in the system. Cases are then listed for staff members to evaluate and ensure that mandated care is provided. With the addition of IWT's CLINICAL ALERTING application, reminders or alerts can be routed to specific caregivers, conveniently ensuring that core measures cases are always handled properly.

COLLECT DATA FOR CORE MEASURES REPORTING

Recording data for core measures cases is easy with technology from IWT. Our software will automatically collect relevant data and use it to pre-populate form fields. Where data is not available electronically, IWT provides data input screens that make recording data accurate and efficient.

EXAMPLE

A potential acute myocardial infarction case is identified by an elevated Troponin lab result. Clinicians are immediately alerted and advised to administer a beta blocker, or provide smoking cessation counseling if it has not been completed. The CORE MEASURES software then gathers appropriate data, such as the left ventricular ejection fraction from echocardiogram results within the past two years. Information that was not available can be entered by staff members so that all of the data is available in one place.
